New Patients:

You may complete the patient history once you have access to the portal or you may print and fill out the history forms here and bring them to your appointment:

New Patient History Form (Complete before appointment)

Please gather any records that are relevant to the reason for your visit and have them faxed to 832-500-5011. Relevant records would include the following:

  • Recent physician notes stating your reason for needing a rheumatology consult
  • Pertinent lab reports
  • Any previous x-rays, MRIs, or other imaging reports (written reports are preferred)
  • Past EMGs/Nerve conduction study results
